Introduction 5.9% of US middle and high school students in 2024 and 6% of adults in 2022 reported electronic cigarette (EC) use in the past 30 days.1 2 The rising prevalence of EC use in the past two decades has generated significant public health and policy interest in understanding consumer demand for ECs and the choices between cigarettes and ECsa harm reduction alternative to cigarettes.36 However, ECs are not harmless because they attract nonsmokers, particularly youth, and may perpetuate nicotine addiction.6 7 Policymakers face a dual challengereducing the appeals of ECs among young people while leveraging their potential in smoking cessation.8 9 Excise taxes on tobacco have been among the most effective policies in curbing tobacco use.10 As of June 2025, 36 states and the District of Columbia (DC) in the USA have imposed excise taxes on ECs.11 Growing evidence further shows that these EC taxes have achieved the desired outcome of decreasing EC use among young people.12 However, increasing EC taxes may lead to unintended consequences of increasing smoking among various populations.13 Meanwhile, states continue to increase cigarette excise taxes, which may provide incentives for smokers to transition to ECs
